For many people, thinking of international medical air transportation brings to mind images of a huge airport in the destination city. This may seem obvious, as large airports are known for extensive infrastructure, multiple runways, and connections all over the globe.
Nevertheless, in medical aviation, the considerations are completely different. The goal of transporting a patient is not just getting there. The main point is safe transfer of the patient, minimizing time spent on the ground and unnecessary movements while ensuring rapid access to the receiving hospital.
That is why many medical aircraft try to avoid using the busiest commercial airports in Europe as far as possible. Terminal congestion, long taxi times, slot issues, and heavy traffic offer little or nothing to patients.
Instead, medical flights often use business and general aviation airports where the aircraft is parked close to the necessary facilities and where the handling teams are used to organising time-critical operations. The selection of the proper airport thus becomes one of the key medical decisions prior to departure.
The closest airport is not always the best airport
The distance does matter, but it is not the only condition. Medical coordinators consider how quickly an ambulance arrives at the airport, whether specialised handling is available 24/7, how swiftly customs and border procedures are completed, and how easily medical teams can move the patient to the hospital.
Sometimes, an airport situated a bit farther, but which avoids traffic congestion, taxi lines, and airline delays could save time in the total transfer of the patient. For critically ill patients, every unnecessary movement should be avoided whenever possible.
Paris: Business aviation often provides the faster solution
Paris illustrates this perfectly. People usually refer to the Charles de Gaulle Airport when they speak about international arrivals.
Nevertheless, for medical aviation, Le Bourget Airport could often be a better option.
Being the busiest business aviation airport in Europe, Le Bourget Airport has special infrastructure suitable for non-scheduled operations. Ambulances can usually access the aircraft directly, the handling teams are familiar with medical flights, and aircraft avoid the congestion typical of major airline hubs.
There is also a financial advantage that is often overlooked. Highly congested commercial airports frequently use higher airport charges to discourage additional traffic, meaning medical aircraft can face significantly higher operating costs.
General aviation airports such as Le Bourget not only provide faster handling and more direct access for patients, but they can also reduce overall airport costs, creating savings that ultimately benefit clients without compromising the quality of care.
Depending on the hospital that receives the patient, this could make the transfer considerably easier.
Though, Charles de Gaulle and Orly Airports continue to play a big role especially for long-haul flights, but they are not necessarily the superior choice for any medical flight.
London: Access depends on the mission
Another example is London. Busy airports like Heathrow process tremendous passenger flow daily. This makes them extremely effective for airline traffic but not so flexible for urgent medical operations.
General aviation airports such as Farnborough, Biggin Hill, and Luton could provide quicker handling for dedicated medical aircraft with great road connections to London’s hospitals.
As in Paris, these airports can also represent a more cost-effective option. Congested commercial airports often charge substantially higher fees as a way of managing demand, whereas dedicated business aviation airports frequently combine faster operations with lower airport charges.
Yet, the aim is never to avoid large airports but is to select the airport that allows the patient to proceed with the fewest operational impediments.
Switzerland: Geography influences every decision
Switzerland demonstrates that airport planning goes far beyond the infrastructure.
Patients requiring treatment in Zurich usually arrive through Zurich Airport, while missions to Geneva prefer to land at Geneva Airport.
But the weather conditions, mountainous geography, runway performance, and seasonality could become quite important in planning.
During the winter months, the alpine weather could change quickly and require alternate airports and flexible planning.
Thus, the medical coordinators evaluate the whole transport chain rather than the destination city only.
Southern Europe brings seasonal challenges
The countries around the Mediterranean have a very sharp increase in air traffic during summer, and popular leisure destinations see a huge increase in private flights alongside airline schedules.

The time difference may be measured in several minutes, but the minutes are crucial when the critically ill patients require uninterrupted medical care.
This is why the flight planners collaborate with the ground ambulance providers, knowing the local traffic situation, hospital procedures, and the quickest transfer routes.
Dedicated handling matters more than passengers realise
Behind every successful medical transfer is an experienced airport handling team.
Unlike commercial passengers, medical flights need ambulances to access the airside area, medical equipment to be moved directly between the aircraft and vehicle, customs services to help with the movement, and special staff to be available instantly throughout the operation.
General aviation airports used for business aviation often perform these tasks more efficiently, as their infrastructure was built for flexibility, not for passengers.
Passengers may not notice these details, but they help minimise delays.
Planning around operational constraints
The airport selection has become even more critical with the continued periods of high demand in European aviation.
Seasonal peaks, parking limitations, runway repairs, and increased business aviation activity could affect which airport an aircraft operates from most efficiently. In some regions, the parking restrictions could mean that the aircraft should be repositioned shortly after the patient’s transfer. Careful coordination is required between the medical teams and the flight operations in this case.
Such operational considerations rarely affect the quality of patient care because the experienced providers can plan them in advance. Rather than depending on one airport, the coordination teams constantly assess the alternative airports that could maintain the schedule while providing rapid access to the receiving hospital.
Flexibility therefore has become as important as familiarity when planning international medical flights.
Every mission has its own solution
In a nutshell, there is no single airport that is always the right choice. The best airport for a medical flight depends on the patient’s condition, destination hospital, aircraft type, weather, operational restrictions, seasonal traffic, and numerous other logistics aspects that occur before take-off.
That is why experienced medical flight providers rarely begin by asking, “Which airport is closest?”
Instead, they ask a much more important question.
“Which airport allows this patient to receive the safest, fastest, and most seamless transfer possible?”
The answer is rarely determined by runway length or passenger numbers alone.
It is determined by how effectively the airport supports the patient’s entire journey, from aircraft door to hospital bedside.
